Numerical analysis of the aortic valve has mainly been focused on the closing behaviour during the diastolic phase rather than the kinematic opening and closing behaviour during the systolic phase of the cardiac cycle. Moreover, the fluid-structure interaction in the aortic valve system is most frequently ignored in numerical modelling. The effect of this interaction on the valve's behaviour during systolic functioning is investigated. The large differences in material properties of fluid and structure and the finite motion of the leaflets complicate blood-valve interaction modelling. This has impeded numerical analyses of valves operating under physiological conditions. A numerical method, known as the Lagrange multiplier based fictitious domain method, is used to describe the large leaflet motion within the computational fluid domain. This method is applied to a three-dimensional finite element model of a stented aortic valve. The model provides both the mechanical behaviour of the valve and the blood flow through it. Results show that during systole the leaflets of the stented valve appear to be moving with the fluid in an essentially kinematical process governed by the fluid motion.  相似文献

A pair of finite element models has been employed to study the interaction of blood flow with the operation of the aortic valve. A three-dimensional model of the left ventricle with applied wall displacements has been used to generate data for the spatially and time-varying blood velocity profile across the aortic aperture. These data have been used as the inlet loading conditions in a three-dimensional model of the aortic valve and its surrounding structures. Both models involve fluid-structure interaction and simulate the cardiac cycle as a dynamic event. Confidence in the models was obtained by comparison with data obtained in a pulse duplicator. The results show a circulatory flow being generated in the ventricle which produces a substantially axial flow through the aortic aperture. The aortic valve behaves in an essentially symmetric way under the action of this flow, so that the pressure difference across the leaflets is approximately uniform. This work supports the use of spatially uniform but temporally variable pressure distributions across the leaflets in dry or structural models of aortic valves. The study is a major advance through its use of truly three-dimensional geometry, spatially non-uniform loading conditions for the valve leaflets and the successful modelling of progressive contact of the leaflets in a fluid environment.  相似文献

Stresses in a prosthetic heart valve at closure are determined by its geometrical and structural characteristics, by the mechanical support environment, and by the momentum of the valve leaflets or occluder and of the blood at the instant of closure. The mass of blood to be arrested is significantly greater than that of the leaflets or occluder, and is therefore likely to dominate the closure impulse. The kinetic energy of the blood must be transduced into potential energy in the structural components (valve leaflets, aortic root and aorta). This paper presents a methodology for computation and parameterisation of the blood momentum associated with a valve in the aortic position. It is suggested that the influence of physiological parameters, such as systolic waveform and systemic impedance, on the closure characteristics can be investigated based on the fluid dynamic implications. Detailed results are presented for a single leaflet mechanical valve (Bjork-Shiley 60 degrees Convexo-Concave). It is demonstrated that a simple analytical method can yield results that might be adequate for the purposes of valve design.  相似文献

Two different aortic prostheses can be used for performing the Bentall procedure: a standard straight graft and the Valsalva graft that better reproduces the aortic root anatomy. The aim of the present work is to study the effect of the graft geometry on the blood flow when a bileaflet mechanical heart valve is used, as well as to evaluate the stress concentration near the suture line where the coronary arteries are connected to graft. An accurate three-dimensional numerical method is proposed, based on the immersed boundary technique. The method accounts for the interactions between the flow and the motion of the rigid leaflets and of the deformable aortic root, under physiological pulsatile conditions. The results show that the graft geometry only slightly influences the leaflets dynamics, while using the Valsalva graft the stress level near the coronary-root anastomoses is about half that obtained using the standard straight graft.  相似文献

The importance of the aortic root compliance in the aortic valve performance has most frequently been ignored in computational valve modeling, although it has a significant contribution to the functionality of the valve. Aortic root aneurysm or (calcific) stiffening severely affects the aortic valve behavior and, consequently, the cardiovascular regulation. The compromised mechanical and hemodynamical performance of the valve are difficult to study both 'in vivo' and 'in vitro'. Computational analysis of the valve enables a study on system responses that are difficult to obtain otherwise. In this paper a numerical model of a fiber-reinforced stentless aortic valve is presented. In the computational evaluation of its clinical functioning the interaction of the valve with the blood is essential. Hence, the blood-tissue interaction is incorporated in the model using a combined fictitious domain/arbitrary Lagrange-Euler formulation, which is integrated within the Galerkin finite element method. The model can serve as a diagnostic tool for clinical purposes and as a design tool for improving existing valve prostheses or developing new concepts. Structural mechanical and fluid dynamical aspects are analyzed during the systolic course of the cardiac cycle. Results show that aortic root compliance largely influences the valve opening and closing configurations. Stresses in the delicate parts of the leaflets are substantially reduced if fiber-reinforcement is applied and the aortic root is able to expand.  相似文献

Clinically significant, unexplained hemolysis has been reported to occur in St Jude Medical (SJM) cardiac valve prostheses. The leakage phase of backflow is identified as having the most hemolytic potential, when compared with the other phases of the cardiac cycle. A two-dimensional, laminar, constant fluid property finite element analysis (FEA) is used to calculate the peak shear stress obtained in the flow through the narrow slit formed by the closed leaflets of an idealized size 29 mm (tissue annulus diameter) SJM cardiac valve prosthesis, during the leakage phase of backflow. The flow geometry was such that the simple laminar flow theory (SLFT) could be used to model the flow through the region of peak shear stresses with reasonable accuracy. The SLFT predicts that the maximum shear stress depends upon the thickness of the clearance space and the average velocity through that space. These results suggest that the magnitude of the peak shear stress is of the order of 700-1000 Pa for a duration of the order of 0.5-0.4 ms for pressure drops across the valve of 150-300 torr. This suggests that hemolysis is possible for certain unfortunate combinations of clearances and pressure conditions. However, further research is needed before this flow phenomenon can be associated with the reported clinical hemolysis.  相似文献

This work was concerned with the numerical simulation of the behaviour of aortic valves whose material can be modelled as non-linear elastic anisotropic. Linear elastic models for the valve leaflets with parameters used in previous studies were compared with hyperelastic models, incorporating leaflet anisotropy with pronounced stiffness in the circumferential direction through a transverse isotropic model. The parameters for the hyperelastic models were obtained from fits to results of orthogonal uniaxial tensile tests on porcine aortic valve leaflets. The computational results indicated the significant impact of transverse isotropy and hyperelastic effects on leaflet mechanics; in particular, increased coaptation with peak values of stress and strain in the elastic limit. The alignment of maximum principal stresses in all models follows approximately the coarse collagen fibre distribution found in aortic valve leaflets. The non-linear elastic leaflets also demonstrated more evenly distributed stress and strain which appears relevant to long-term scaffold stability and mechanotransduction.  相似文献

Failure of synthetic heart valves is usually caused by tearing and calcification of the leaflets. Leaflet fiber-reinforcement increases the durability of these valves by unloading the delicate parts of the leaflets, maintaining their physiological functioning. The interaction of the valve with the surrounding fluid is essential when analyzing its functioning. However, the large differences in material properties of fluid and structure and the finite motion of the leaflets complicate blood-valve interaction modeling. This has, so far, obstructed numerical analyses of valves operating under physiological conditions. A two-dimensional fluid-structure interaction model is presented, which allows the Reynolds number to be within the physiological range, using a fictitious domain method based on Lagrange multipliers to couple the two phases. The extension to the three-dimensional case is straightforward. The model has been validated experimentally using laser Doppler anemometry for measuring the fluid flow and digitized high-speed video recordings to visualize the leaflet motion in corresponding geometries. Results show that both the fluid and leaflet behaviour are well predicted for different leaflet thicknesses.  相似文献

Wall shear stress has been measured by flush-mounted hot film anemometry distal to an Ionescu-Shiley tri-leaflet valve under pulsatile flow conditions. Both Newtonian (aqueous glycerol) and non-Newtonian (aqueous polyacrylamide) blood analog fluids were investigated. Significant differences in the axial distribution of wall shear stress between the two fluids are apparent in flows having nearly identical Reynolds numbers. The Newtonian fluid exhibits a (peak) wall shear rate which is maximized near the valve seat (30 mm) and then decays to a fully developed flow value (by 106 mm). In contrast, the shear rate of the non-Newtonian fluid at 30 mm is less than half that of the Newtonian fluid and at 106 mm is more than twice that of the Newtonian fluid. It is suggested that non-Newtonian rheology influences valve flow patterns either through alterations in valve opening associated with low shear separation zones behind valve leaflets, or because of variations in the rate of jet spreading. More detailed studies are required to clarify the mechanisms. The Newtonian wall shear stresses for this valve are low. The highest value observed anywhere in the aortic chamber was 2.85 N/m2 at a peak Reynolds number of 3694.  相似文献

A finite element scheme has been developed using total Lagrangian techniques for the two-dimensional analysis of bioprosthetic heart valve leaflets undergoing large deformation. Two models of a leaflet, namely a radial and a circumferential slice, have been analysed. The attachment of the slice to the stent was simulated by progressive contact on a circular former and the coaptation of the leaflets in the centre of a heart valve by a straight line of contact. For the circumferential model, different initial configurations have been considered. The prolapse pressure under which the heart valve closes has been shown to be small in comparison with the normal pressure a heart valve sustains. The regions of the valve that are most heavily stressed are subjected to a strong component of bending. The amount is sensitive to the details of the boundary conditions and to the initial configuration of the valve. These observations are likely to be significant in the use of this kind of stress analysis to improve the design of this type of valve.  相似文献

Edge-to-edge technique is a surgical procedure for the correction of mitral valve leaflets prolapse by suturing the edge of the prolapsed leaflet to the free edge of the opposing one. Suture presence modifies valve mechanical behavior and orifice flow area in the diastolic phase, when the valve opens and blood flows into the ventricle. In the present work, in order to support identification of potentially critical conditions, a computational procedure is described to evaluate the effects of changing suture length and position in combination with valve size and shape. The procedure is based on finite element method analyses applied to a range of different mitral valves, investigating for each configuration the influence of repair on functional parameters, such as mitral valve orifice area and transvalvular pressure gradient, and on structural parameters, such as stress in the leaflets and stitch tension. This kind of prediction would ideally require a coupled fluid-structural analysis, where the interactions between blood flows and mitral apparatus deformation are simultaneously considered. In the present study, however, an alternative approach is proposed, in which results obtained by purely structural finite element analyses are elaborated and interpreted taking into account the Bernoulli type equations available in literature to describe blood flow through mitral orifice. In this way, the effects of each parameter in terms of orifice flow area, suture loads, and leaflets stresses can be expressed as functions of atrioventricular pressure gradient and then correlated to blood flow rate. Results obtained by using this procedure for different configurations are finally discussed.  相似文献

A numerical method is developed for simulating unsteady, 3-D, laminar flow through a bileaflet mechanical heart valve with the leaflets fixed. The method employs a dual-time-stepping artificial-compressibility approach together with overset (Chimera) grids and is second-order accurate in space and time. Calculations are carried out for the full 3-D valve geometry under steady inflow conditions on meshes with a total number of nodes ranging from 4 x 10(5) to 1.6 x 10(6). The computed results show that downstream of the leaflets the flow is dominated by two pairs of counter-rotating vortices, which originate on either side of the central orifice in the aortic sinus and rotate such that the common flow of each pair is directed away from the aortic wall. These vortices intensify with Reynolds number, and at a Reynolds number of approximately 1200 their complex interaction leads to the onset of unsteady flow and the break of symmetry with respect to both geometric planes of symmetry. Our results show the highly 3-D structure of the flow; question the validity of computationally expedient assumptions of flow symmetry; and demonstrate the need for highly resolved, fully 3-D simulations if computational fluid dynamics is to accurately predict the flow in prosthetic mechanical heart valves.  相似文献

We carry out three-dimensional high-resolution numerical simulations of a bileaflet mechanical heart valve under physiologic pulsatile flow conditions implanted at different orientations in an anatomic aorta obtained from magnetic resonance imaging (MRI) of a volunteer. We use the extensively validated for heart valve flow curvilinear-immersed boundary (CURVIB) fluid-structure interaction (FSI) solver in which the empty aorta is discretized with a curvilinear, aorta-conforming grid while the valve is handled as an immersed boundary. The motion of the valve leaflets are calculated through a strongly coupled FSI algorithm implemented in conjunction with the Aitken convergence acceleration technique. We perform simulations for three valve orientations, which differ from each other by 45 deg and compare the results in terms of leaflet motion and flow field. We show that the valve implanted symmetrically relative to the symmetry plane of the ascending aorta curvature exhibits the smallest overall asymmetry in the motion of its two leaflets and lowest rebound during closure. Consequently, we hypothesize that this orientation is beneficial to reduce the chance of intermittent regurgitation. Furthermore, we find that the valve orientation does not significantly affect the shear stress distribution in the aortic lumen, which is in agreement with previous studies.  相似文献

This work presents a numerical simulation of intraventricular flow after the implantation of a bileaflet mechanical heart valve at the mitral position. The left ventricle was simplified conceptually as a truncated prolate spheroid and its motion was prescribed based on that of a healthy subject. The rigid leaflet rotation was driven by the transmitral flow and hence the leaflet dynamics were solved using fluid-structure interaction approach. The simulation results showed that the bileaflet mechanical heart valve at the mitral position behaved similarly to that at the aortic position. Sudden area expansion near the aortic root initiated a clockwise anterior vortex, and the continuous injection of flow through the orifice resulted in further growth of the anterior vortex during diastole, which dominated the intraventricular flow. This flow feature is beneficial to preserving the flow momentum and redirecting the blood flow towards the aortic valve. To the best of our knowledge, this is the first attempt to numerically model intraventricular flow with the mechanical heart valve incorporated at the mitral position using a fluid-structure interaction approach. This study facilitates future patient-specific studies.  相似文献

Glutaraldehyde-treated bovine pericardium is used successfully as bioprosthetic material in the manufacturing of heart valves leaflets. The mechanical properties of bovine pericardial aortic valve leaflets seem to influence its mechanical behaviour and the failure mechanisms. In this study the effect of orthotropy on tricuspid bioprosthetic aortic valve was analysed, using a three-dimensional finite element model, during the entire cardiac cycle. Multiaxial tensile tests were also performed to determine the anisotropy of pericardium. Seven different models of the same valve were analysed using different values of mechanical characteristics from one leaflet to another, considering pericardium as an orthotropic material. The results showed that even a small difference between values along the two axes of orthotropy can negatively influence leaflets performance as regard both displacement and stress distribution. Leaflets of bovine pericardium bioprostheses could be manufactured to be similar to natural human heart valves reproducing their well-known anisotropy. In this way it could be possible to improve the manufacturing process, durability and function of pericardial bioprosthetic valves.  相似文献

A transient fluid–structure interaction (FSI) model of a congenitally bicuspid aortic valve has been developed which allows simultaneous calculation of fluid flow and structural deformation. The valve is modelled during the systolic phase (the stage when blood pressure is elevated within the heart to pump blood to the body). The geometry was simplified to represent the bicuspid aortic valve in two dimensions. A congenital bicuspid valve is compared within the aortic root only and within the aortic arch. Symmetric and asymmetric cusps were simulated, along with differences in mechanical properties. A moving arbitrary Lagrange–Euler mesh was used to allow FSI. The FSI model requires blood flow to induce valve opening and induced strains in the region of 10%. It was determined that bicuspid aortic valve simulations required the inclusion of the ascending aorta and aortic arch. The flow patterns developed were sensitive to cusp asymmetry and differences in mechanical properties. Stiffening of the valve amplified peak velocities, and recirculation which developed in the ascending aorta. Model predictions demonstrate the need to take into account the category, including any existing cusp asymmetry, of a congenital bicuspid aortic valve when simulating its fluid flow and mechanics.  相似文献
